The Hypertension Canada Professional Certification Program (PCP) is designed to arm primary healthcare professionals with the knowledge and confidence to identify and manage patients with hypertension.
The PCP is no longer accepting new registrations.
If you are already enrolled, you may continue working through the program. To claim CCCEP credits, the program must be completed by May 6, 2026, which reflects the end of the accreditation period. Participants who do not require CCCEP credit may continue accessing the program content until September 1, 2026.
We are developing new educational programming based on the 2025 Guidelines and encourage you to watch for updates on our website and social media.
Graduates of the program can use the post-nominal letters HC-CHM (Certified in Hypertension Management) after their name for the duration of the certification.
This course helps primary care providers build the confidence to identify and manage patients with hypertension.
The course utilizes practical application and skill-building techniques to help providers implement hypertension management strategies.
This course provides evidence-based learning with the goal to improve guidelines adoption and adherence.
